Adaptive Sports pros who are currently full-time, year-round employees of an Adaptive Sports Program for individuals with physical and cognitive disabilities may renew by submitting the following documentation:

• Current Customer Number
• Company Work Email
• Company brochure or website link detailing your operation, program and services
• Recent pay stub that indicates full-time status (Black out any sensitive information).
